Substance Abuse Evaluations
This type of evaluation is used to examine an individual’s substance use history, determine the extent of their substance use disorder (if any), and provide a recommendation for the level of care a client needs. Evaluations may be requested by attorneys, courts, probation officers, or employers. This appointment will generally last one hour and the client will participate in an interview and complete two screening forms along with general paperwork. The completed evaluation will only be given to the individual authorized by a release of information signed by the client. The current cost of a substance use evaluation is $150 and is due at the time of service. Completion and delivery time is generally 7-10 days. Expedited times of 5 days or less are available for an additional fee of $150.

Secretary of State Evaluations
This type of evaluation is provided for individuals who are attempting to restore their driving privileges in the State of Michigan. As qualified evaluators, we will conduct a comprehensive interview that will cover any substance use history, abstinence history, substance related arrests, substance use disorder treatment history, and self-help group (AA/NA, SMART, Celebrate Recovery, etc.) attendance. This evaluation is not a determination, but instead is one piece of evidence used by the State, along with the individual’s self-report, urinalysis results, letters of attestation from supporters, and questions and testimony gathered at an administrative hearing conducted by the State of Michigan. The current fee for this type of evaluation is $300 and is due at the time of service. This evaluation is valid for 90 days after the date of service.
